投标人应具备的资格或业绩：1、法定代表人授权书；2、投标人营业执照或企业注册证明复印件；3、《医疗器械生产企业许可证》或《医疗器械经营企业许可证》（提供扫描件，原件备查）；4、所投产品必须具有中华人民共和国医疗器械注册证（提供扫描件，原件备查）；5、产品制造商或合法代理商或制造商针对本项目直接授权的供应商（提供相关证明扫描件，原件备查）；6、投标人必须在中国国际招标网（ www.chinabidding.com）注册成会员。境内投标人还须在深圳市政府采购网注册成会员；7、近三年内无行贿犯罪记录（由深圳市政府采购中心定期向深圳市人民检察院申请对政府采购供应商库中注册有效的供应商进行集中查询，投标文件中无需提供证明材料）；8、本项目不接受联合体投标。 |
